Continuum Care can assist an individual or family to plan their services but this will always be based on what is important to the person receiving any support. Services or informal supports are designed to support the individual’s lifestyle.

Our experienced team are all qualified practitioners who are able to work closely with Customers and their families to find and provide the support they want.

Our Support Team acknowledge that the customer is the expert in their own life and by working to meet the customers goals is the way to craft the best outcome.

Customers are in control of the processes and the decision making. Customers decide what services they want and wherever possible, who they want to provide services. Continuum Care becomes a partner to enact and develop a plan that builds our customer’s goals and preferences.

Services for You

We can support you with the following supports:

Accommodation/Tenancy

  • Assistance provided to guide, support and prompt participants, or to undertake on a participant's behalf, activities to ensure they obtain or retain appropriate accommodation

Assist Personal Activities

  • Assisting with, and/or supervising, personal tasks of daily life to develop skills of the participant to live as autonomously as possible. These supports are provided individually to participants. The support can be provided in a range of environments.

Personal Activities High

  • Assisting with, and/or supervising, personal tasks of daily life to develop skills of the participant to live as autonomously as possible. These supports are provided individually to participants. The support can be provided in a range of environments.

Community Nursing Care

  • Provision of nursing care to promote physical well-being including preventive monitoring of ongoing conditions and provision of direct treatments not able to be undertaken by the person as a result of their disability. This is not acute nursing care as delivered by the health system.

Daily Tasks/Shared Living

  • Assisting participants to develop the social supports, daily living and life skills they need to successfully manage an arrangement in which two or more people share a house or an apartment (this can also mean living with others in their own or a relatives home), as well as participate in community, social and civic activities.

Community Participation

  • Assisting participants to participate actively in community, social and civic activities by developing and enhancing a participants skills and providing supports during these activities.

Development-Life Skills

  • Training and development activities undertaken by a participant or their carer to increase their ability to live as autonomously as possible. Training can be provided either individually or through group activities and can include skills in daily life activities, communication and social skills, problem solving and managing funding of supports.

Household Tasks 

  • Supports to enable the participant to maintain their home environment. This may involve undertaking essential household tasks or the provision of support that the participant is not able to undertake.

Therapeutic Supports

  • Therapeutic supports to assist the participant to gain skills and improve independence related to identified needs in areas such as language and communication, personal care, mobility and movement, interpersonal interactions, community living to support positive changes in the person's functioning, development and well-being.
  • Therapeutic supports are delivered by professionals including Occupational Therapy, Physiotherapy, Speech and Language Pathology and Psychology or by a therapy assistant under the supervision of the therapist.

Support can be short term, long term or once off.

What is the National Disability Insurance Scheme?

The National Disability Insurance Scheme (NDIS) is a national scheme designed to improve disability services in Australia. The NDIS supports people with a permanent and significant disability that affects their ability to take part in everyday activities.

The NDIS will enable you to pay for supports that may help you achieve your goals in many aspects of your life, including independence, involvement in your community, education, employment and health and wellbeing.

The NDIS gives you more choice and control over how, when and where your supports are provided, and gives you certainty you will receive the support you need over your lifetime.

Currently the NDIS is available for children aged 13 years and under, however, the NDIS will be operating state-wide for all age groups by July 2018. Between 1 July 2017 - 30 June 2018, Adults aged 18–64 years old will move to the NDIS at different times depending on their age and where they live.  Below is a list of the proposed NDIS rollout in South Australia for adults.

 

What does this mean for you?

  • You will have much more choice and control over the supports and services you receive.
  • Each individualised funding package allows you the right to choose the service agency or agencies you want to provide your identified supports. You are free to change service providers who do not adequately and satisfactorily meet your needs.
  • You can choose either to directly employ your support worker and/or negotiate your supports from providers of support workers.
  • You can get support from intermediaries in making choices and handling administrative tasks, or you can choose to manage you own package and purchase your own supports.
  • As your individual circumstances change, you will be able to apply to have your funding plan adjusted accordingly.

Our aim is to provide flexible and affordable services to customers - delivered by experienced, qualified professionals who love their work, are committed to providing high standards of care. We take the time to ensure our services match your decisions and your individual needs.

  • All workers have been carefully screened for qualifications, police checks and references.

 Continuum Care’s team of workers are Support Specialists who comprise, professionals including nurses, social workers, support workers and other independent contractors who are experienced and specialise in supporting people across the community.

Continuum Care also have access to a network of professionals such as bankers, gardeners, electricians, accountants, lawyers, chiropractors, exercise physiologists as well as other professionals and tradies you can trust.

NDIS Rollout in South Australia

Northern Adelaide

The NDIS will become available for adults aged 18 to 64 years in the City of Playford, City of Salisbury and City of Port Adelaide Enfield (East) local government areas from 1 July 2017. The NDIS will become available for adults aged 18 to 64 years in the City of Tea Tree Gully local government area from 1 October 2017. The Northern Adelaide region covers the local government areas of:

  • City of Playford
  • City of Salisbury
  • City of Port Adelaide Enfield (East)
  • City of Tea Tree Gully

Southern Adelaide

The NDIS will become available for adults aged 18 to 64 years in the Southern Adelaide region from 1 January 2018. A person may meet the access requirements to become a participant of the NDIS up to 6 months prior to this date. The Southern Adelaide region covers the local government areas of:

  • City of Holdfast Bay
  • City of Marion
  • City of Mitcham
  • City of Onkaparinga

Western Adelaide

The NDIS will become available for adults aged 18 to 64 years in the Western Adelaide region from 1 April 2018. A person may meet the access requirements to become a participant of the NDIS up to 6 months prior to this date. The Western Adelaide region covers the local government areas of:

  • City of Charles Sturt
  • City of West Torrens
  • City of Port Adelaide Enfield (West)

Eastern Adelaide

The NDIS will become available for adults aged 18 to 64 years in the Eastern Adelaide region from 1 April 2018. A person may meet the access requirements to become a participant of the NDIS up to 6 months prior to this date. The Eastern Adelaide region covers the local government areas of:

  • City of Adelaide
  • City of Burnside
  • City of Campbelltown
  • The City Of Norwood, Payneham & St Peters
  • City of Prospect
  • City of Unley
  • Corporation of the Town of Walkerville

Southern Adelaide

The NDIS will become available for adults aged 18 to 64 years in the Southern Adelaide region from 1 January 2018. A person may meet the access requirements to become a participant of the NDIS up to 6 months prior to this date. The Southern Adelaide region covers the local government areas of:
