Whether you need support with cleaning your home, meal preparation or assisted shopping.
We can arrange gardening or maintenance services to ensure your safety in and around your home. These services also include tasks such as changing lightbulbs, installing emergency alarms and routine repairs.
Personal Care
Personal Care can assist customers with physical impairments as well as customers with cognitive impairments, such as Alzheimer’s or mental illness. Personal Care includes tasks related to hygiene, such as bathing, brushing teeth, going to the toilet, transfers to and from a lifter or wheelchair. Personal Care also includes, organising a customer’s schedule, planning appointments and arranging transport to doctors appointments or the chemist. Assistance with shopping for personal items and groceries or helping to prepare and plan meals can keep a person in touch with their community and companionship through talking with the customer, playing games or going for walks can assist a person to live in their own home and community longer.

We offer our support though positive thinking, relaxation, promoting a healthy diet and regular exercise. We can have one of our friendly staff members visit you at your home and spend time with you doing the things you love.
We can also assist in arranging outings and accompanying you on these (as needed).
Nursing Services
Transitioning home from a hospital stay can be a daunting thought but with help our nursing services we can make it a smooth and stress-free process.
Our qualified nursing staff will assist you in managing your health conditions from home. Some of our nursing services include dressing changes, application of medication and stoma/colostomy management.

Securing your future
Continuum Care welcomes self-funded customers as well as customers with approval through the Government funded ‘My Aged Care’ and the Aged Care Assessment Team (ACAT). As an approved Home Care provider, we are able to provide a tailored and flexible Home Care Package that supports your choice about how you want to live.
If you have not yet contacted My Aged Care or had an ACAT assessment completed, or you are not sure of what you are eligible for, we can assist to facilitate this ensuring a smooth and stress-free process. This is a FREE service we provide.
